We accept referrals from Individuals, Local Authorities and Clinical Commissioning Groups (CCG’S).

Referrals

If you think that you or someone you know would benefit from our support, contact your local service. We work closely with local authorities and CCG’s who will be able to provide information and advice on how you can apply for support as well as your eligibility.

Some referrals can be accepted from individuals in receipt of direct payments, personal budgets or private customers. However, we encourage the involvement of a representative from the health / social care sector is sought.

The referral process can be a daunting and sometimes confusing period. However, at Comfort Living Care we want individuals to be reassured that as a service we will work closely with you and your support network at every stage.

We review all referrals and undertake assessments to ensure that Comfort living Care is the right service to provide you support and meet your needs. We also want to ensure you are  empowered to have choice and control over who supports you and how you are supported.

Any enquiries about vacancies, accessing support or general information about the organisation can be made via telephone, email or via our website. Once received these will be directed to the registered manager/ senior support worker who have responsibility over the referral process and completion of screening and referral assessments.
